Do I need a doctor’s referral to receive treatment?
For most insurance, the answer is no. Direct Access means you can visit a physical therapist without a prescription or referral. Individuals can go to a physical therapist of their choice and receive the treatment they need. NY State allows 10 visits within 30 days before requiring a prescription. An exception to this is patients insured by Medicare. They do need a prescription to receive physical therapy treatment.

How long is each appointment going to be?
You can expect the initial evaluation to last about 75 minutes. After that, each visit will last approximately 60 minutes, but we can work with your schedule. Most patients see the therapist 2-3 times a week.
What do I need to bring to my first appointment?
You will need to bring your insurance card and prescription, if you have one. For Workers' Compensation and No-Fault cases, please have the name of the carrier, the adjuster's name and contact number, as well as your carrier number. There is the option to complete the new patient paperwork online through a link that is sent via email. If this is not convenient for you, then please arrive a few minutes early to complete the paperwork in the office prior to your visit.
What clothing should I wear to physical therapy?
Please wear comfortable clothing that allows the therapist to access the injured body part. We have changing areas for patients if needed.
How can I pay for any balance I may owe?
We accept cash, check or credit cards including HSA cards.
What are your hours?
Ascend Physical Therapy is open Monday-Thursday 8am-8 pm and Friday 8-6pm. Saturday hours vary, please call the office at (516) 742-4222 for more information.
